From the lyrics I always pictured the old man as being in a dying condition not already deceased. The fact that the lyrics are sad would work in either case.

Also, be sure to search for the album cover of Sail Away in which he sharpens his nails with a butcher knife.
You could not have misunderstood the song any deeply than you did.
It's about a son saying goodbye to his already deceased dad. It's evident by the sad lyrics that the son wasn't there with him when his dad died, and this is a very touching and candid eulogy, so beautiful it could only have been written by Randy Newman.
